         *Let you know the costs of your treatment, and refer you to our eligibility specialist, if needed.

         *Provide a safe way to voice concerns you may have about your treatment.

 


 

Client Rights

 

                 *Considerate, respectful and ethical care at all times.

                 * Fully participate in all decisions regarding your treatment. If you are unable to participate in treatment decisions, you have the right to be represented by
              parents, guardians, family members or other conservators.

                 * Have information concerning your treatment program, including alternatives.

                 * Request a re-evaluation of treatment procedures.

           * Refuse treatment to the extent permitted by law after being informed of all the consequences associated with your choice.

                 *Involve and include family members or friends in your treatment.

                 *Expect your record to be kept confidential, except where law requires disclosure.

                 *Have your bill explained or charges examined.

                 *Request a re-evaluation of fees, due to a change in income.

 


 

 Client Responsibilities

 

 

                        *Accept responsibility for your recovery and living a healthy lifestyle.

                   *Actively participate in making decisions regarding your treatment and recovery plan.

                   *Learn as much as you can about your mental illness.

                   *Inform your clinician if you are having problems working on your treatment goals.

                   *Learn and practice new behaviors that will assist you in achieving your goals.

                   *Ask questions when you don’t understand something.

                   *Show respect for property and the rights of others.

                   *Be familiar with and observe the rules of the Center.

                   *Do not come to the Center under the influence of alcohol, illicit or street drugs.

                   *Do not bring weapons to the Center.

                   *Do not make any verbal or physical threats against another person.
